An elderly man, smoker, presented with cough and weight loss. A chest-ray revealed a small right lung nodule. CT scan thorax revealed a small right lung nodule, likely a cancer and mediastinum nodes. It was felt that FNAC of the mediastinum nodes would give a better yield that FNAC of the lung nodule. The node was targeted easily and cytology reported non-small cell adenocarcinoma.
